Description
Good day players, members and affiliates,
 
Please assist the association by completing the following registration. All players participating in Masters leagues organised by SGMHA, as well as players seeking to represent Southerns Masters at IPT, are required to register.
 
As SGMHA. is affiliated to South African Masters Hockey Association, who is in turn affiliated to South African Hockey (SAHA), we are compelled to comply with the constitution and bylaws of these organisations. As such, we are expected to ensure that all members/players are suitably registered with SAHA. Please make sure to have your SAHA membership number available to submit with this registration.  
 
2025 annual player registration fee:
  • SGMHA member (applies to SHINS, Over45s, Mixed or SG IPT teams): R190 
  • Non-member - Northerns, Easterns, etc. (applies to SHINS, Over45s, Mixed): R120
  • Grand Masters (applies to players only participating in Grand Masters): R120
  • Players 70yrs and older: Fully waived. 
Please note the rates above do note take into account any fees due to national bodies, such as South African Hockey Associaiton or South African Masters/Grand-Masters Hockey Association(s). 
 
Payments can be made to SGMHA bank account or via payment gateway at the end of this form.

I waive my rights to claim against any SOUTH AFRICAN MASTERS HOCKEY ASSOCIATION (SAMHA), SOUTHERN GAUTENG MASTERS HOCKEY ASSOCIATION (SGMHA) or the host venue, their officers, members, coaches, managers, players, servants and agents, and indemnify the same in respect of all claims arising from or connected with injury, disability, death or loss which I might sustain while, or as a result of, participating in the Masters Hockey and/or organised team practices for the tournament or related activity, whether caused by negligence or otherwise.
